What do I need to bring to my medical appointment?
We will be pleased to bill the above insurance carriers on your behalf. Copays and deductibles are the responsibility of the patient. Your copay will be collected at the time of your visit. Please be sure to bring your Insurance Card to your medical visits to ensure proper billing.

What if my insurance carrier is not listed above?
If you have other insurance carriers, you will be expected to pay for your service in full. We will prepare a superbill which you may then send to your insurance carrier for reimbursement.

Cosmetic Procedures and Insurance
Most all Cosmetic Procedures are NOT covered under insurance plans and must be paid in full at the time of service. We contract with Care Credit for those patients who wish to take advantage of consumer credit. Applications are processed immediately and you may do this at our office or you may apply online and establish your account prior to your visit. Ask our staff about current interest offers for specific time periods. Care Credit Finance
