The majority of today’s cosmetic fillers are biodegradable and temporary. They eventually break down and leave the body through natural processing. Their ease of use, safety profile and patient satisfaction have kept them a highly desirable cosmetic option.
Permanent fillers, on the other hand, stay in the skin permanently and the body cannot break them down. Due to the dispersion nature of fillers, permanent fillers must be considered an irreversible decision as surgical removal attempts are very complex and sometimes impossible.
Currently in use are two permanent products. One is the FDA approved filler Bellafill. Another permanent “filler” product is silicone oil – which is not FDA approved for cosmetic use for wrinkles or any other purpose outside of eye surgery for detached retinas.
Granted, all of us want to get the most for our dollars and permanent fillers can seem like a huge money saver over the long term; but they do come with some levels of risk that do not exist with fillers like Juvederm, Restylane or Belotero, all of which can be simply and easily dissolved if one doesn’t like the effect… Another bigger issue is that temporary fillers can be placement-adjusted as we age and as the skin, bones and muscle change, shift and rearrange.
We continue to age despite any intervention and that aging actually changes the shape and size of the bony support structures; muscles become more lax and elongated; fat naturally shifts and dissipates and the skin loses elasticity. Those are the VERY reasons non-permanent fillers are, in my opinion, the best option.
*The exception to the use of Bellafill is in the use as a scar filler. Since placement is very superficial, there is not a similar concern as with fillers used deeper in the skin. Bellafill is FDA approved for acne scars.
The adjustment of filler placement can then be addressed at each event to fit the changing landscape of the face; thus allowing a custom “fit” for the new and ever-changing landscape of the face.
