For medical office visits, we are a cash-only practice. Our initial medical office visit is $275 and follow-up medical appointments are $175. Please feel free to contact us at the number listed below if you have any questions, or would like to schedule an appointment.
**AS OF JANUARY 1, 2017, WE WILL NO LONGER BE CONTRACTED WITH ANY MEDICAL INSURANCE COMPANIES.
All medical office visits will be self-pay. If you choose to submit a claim to your insurance company on your own, we will provide you with the appropriate paperwork; however, because we are considered an out-of-network provider, reimbursement is not guaranteed, and out of pocket patient responsibility may vary.
Cosmetic Procedures are NOT covered under insurance plans and must be paid in full at the time of service.
We contract with Care Credit for those patients who wish to take advantage of consumer credit. Applications are processed immediately and you may do this at our office or you may apply online and establish your account prior to your visit.
